Pre-Selecting and notifying of Profilee and Profilers

This form gives you the ability to select individuals to be profiled and to select people to profile the individuals.

1: Simply enter YOUR name and email address

2: Enter the name & email address of the individual to be PROFILED

3: Enter the names, email addresses and there relationship to profilee i.e. Leader, Colleague & Direct Report.

Once you have done this click Submit and an email will be sent to all parties asking them to go to the Questionaire and fill it out.
