Self selection of Profilers
This form gives you the ability to select individuals and to ask them to answer the profile questionnaire on you.
Simply give your name and email address then enter the email addresses of individuals that you want to score you giving there relationship to you i.e.: Leader, Colleague & Direct Report. Once you have done this click Submit and an email will be sent to all parties asking them to go to the Questionnaire and fill it out
